PLEDGE CARDS - The Pledge card is now a 3 part form so if you decide to download it, please be sure to print 3 copies, 1 for the donor, 1 for the company and 1 for United Way.  The back of the card also goes to the donor, so please be sure to print that piece.  You can download a .pdf copy or contact the office, 608-796-1400, for pre-printed forms.

POSTER/BROCHURE - The brochure and poster have been combined this year into a 2-sided color document.  1 side is a poster listing the "Top 10 Reasons to Invest in Your Community" and the other side would be suitable as either a handout or poster explaining the impact of your contribution and why United Way is a sound investment of your donated dollars.  You can download the .pdf copy or contact the office, 608.796.1400 for pre-printed forms
